Los Angeles County Aging & Disabilities Department (AD) is releasing a Request for Proposals (RFP) for Linkages Program Services. The primary purpose of the Linkages Program is to provide comprehensive care management that serves and links individuals with a full range of appropriate services. The goal of the Linkages Program is to prevent premature or inappropriate institutionalization of at-risk individuals by promoting and maintaining independent living for frail adults, adults with disabilities, and residents of residential care facilities who are scheduled to be discharged within 30 to 60 days from the date of the referral for Program Services.
